PASSION GROUP HOME offers residential care and treatment to individuals between the ages of 18 and above with developmental disabilities. Our group home provides high-quality care for mentally challenged individuals with a wide array of services based on their medical needs, levels of functioning, and supervision requirements.

An on-site licensed practical nurse (LPN) is available 24/7 to ensure that care plans are being followed and to help administer medications.

Adult residential services

We help meet the needs of individuals who seek community placement in group homes with adequate staff, appropriate equipment, and quality amenities. These homes allow optimal growth and development.

Community living

The care and treatments plans that we follow are customized according to each resident’s needs, preferences, and interests. These personalized plans are enhanced by case management and patient advocacy.

Individual support services

We provide a plethora of support services to residents with chronic medical conditions and developmental or intellectual disabilities.

Community-supported living arrangements

We provide a plethora of support services to adults with chronic medical conditions or developmental/intellectual disabilities including outreach assistance, 24/7 in-home assistance, socialization encouragement, and nurse monitoring.

Individual and family support services

We provide support services that are specifically designed to cater to the needs of individuals with developmental disabilities, as well as their caregivers and families. We offer patient advocacy, education, resources, and short-term respite care.
